The Personal Transition Plan is a powerful new tool to help our students build a portrait of themselves that reflects their goals and aspirations in life and how their high school experience will prepare them for this adventure. It is meant to be the foundation of a resumé that will grow with their experiences and accomplishments, both during and beyond their high school years.

To satisfy this graduation requirement and receive the half credit, students at Konawaena High School can use two web based programs. The first is e-Portfolio, which has been in use for the past four years. The newer program, Navigation101, was added last year and includes a five lesson course, a Portfolio page, and a My Plan page. (Your PTP can be done in a conventional hard copy format as well)

The Common (College) Application is a not-for-profit organization that serves students and over 300 member institutions by providing an admisssion application online and in print that students may submit. Many well known colleges and universities use this form.

For students using the e-Portfolio system, a YES check-list is provided. For those using the Navigation101 program, you need to complete the five lessons, the portfolio, and my plan.

For all seniors, the personal statement is required and a guide is provided below. The personal statement can then, be uploaded to either program. Requirements 2, 3, and 4 are accomplished on the “Portfolio” page and then summarized on the “My Plan” page.

When your transition plan is complete or if you need assistance, see Mr. Heinecke in R105 for a final check.
